The Ministry of Health has made arrangements to issue COVID-19 Smart Vaccination Certificates (SVC) to those who need proof of vaccination when travelling abroad.
The SVC can be obtained by any person who has completed the relevant vaccination regime approved by the Epidemiology Unit of the Ministry of Health.
According to Health Services Director General Dr. Asela Gunawardena, the SVC can be generated by the COVID-19 vaccination tracker information system currently implemented in MOH offices countrywide.
A person who requires the SVC is advised to make a request to his or her residing MOH office in order to obtain the certificate. A printed or electronic version of the certificate can be generated through the COVID-19 vaccination tracker information system. However, the personal immunisation record should be completed in the system to generate the certification.
The MOH should verify the details of the vaccine recipient through the National Identity Card/Passport. If there are minor changes to be made to the name/address, such as spelling mistakes/incorrectly entered name, such details should be amended by the MOH, and the vaccine certificate issued.
Gunawardena has circulated the user guide for the MOH to generate and issue the SVC to all provincial and regional directors of health services, as well as all medical officers of health.
The detailed information regarding the SVC issuing process can be referred through the COVID-19 vaccination tracker information system. All relevant directors and officers have been requested to take immediate action to ensure full implementation of issuing the SVC through the MOH offices in their areas.
